* gateway/mcp: richer resolver Call + spec-correct HTTP handler Resolver.Call now returns (*CallResult, error): a tool that ran but failed sets CallResult.IsError (returned as a tools/call result with isError:true, per the MCP spec), while a protocol/pre-check failure returns an error -- an *RPCError carries a specific JSON-RPC code.
